How to give Vaman ?

Vaman (Therapeutic Vomition ) –

Pre treatment of Vaman 

      Firstly examine the patient carefully whether he is fit for vamana or not.

       Patient should be mentally prepared for the treatment.

      Then better written consent should be taken .Although procedure is safe ,if done it in a scientific manner & by experienced vaidya .But still considering very rare complications like Cardio Pulmonary arrest or Haemoptysis or Haemetamesis or Dehydration ,it is advisable to take written consent & better this procedure should be done in hospital set up ,where emergency if at all occurs ,can be managed easily

      If the patient is fit for vaman then start pretreatment i.e. snehana (internal and external Oleation) and Swedana (fomentation) as stated earlier.

      Examination of digestive power can be done by giving the testing dose of oily substances to the patient. This testing dose is termed as ‘Rhasiyasi matra’ of Snehana. Time taken by the patient to digest this dose decides the proper dose. This testing dose is generally of 25gm.

      So mathematical Equation will be as follows

      If Patient is takes 3 hours for Digestion of 25 Grams ,then How much will be quantity to Digest in 6 hours ? Answer will be 50  Grams ! This will be Rhaswa or Small dose.

      100 Gm will be digested in 12 hours .This is Madyam or Moderate dose .
      200 Gm will be digested in 24 hours .This will be Uttam or High Dose.
      The quantity of sneha to be taken is increased gradually for 3- 5-7 days. 

On the next day of Samyak ,adequate & complement Snehapan i.e  completion of internal oleation, next day external oleation and sudation is given. On the next day Vaman is given with prior external oleation and sudation. It means for vaman there is only one day break.

      After the appearance of the symptoms of proper Oleation, internal Oleation should be stopped.
      Give one day break and on the next day one should go for the process of vaman.
      During these two days massage and fomentation should be continued.
      During the period of pretreatment patient should be examined regularly.

Main procedure of Vaman 

      The dinner on the day before of Vaman ,Kapha vardhak or Kapha increasing diet is advised like Udid or black gram vada with curd or Rice & curd  ,Rice with milk,Banana, Sadha Rawa dosa without curry etc

      It increases kapha, kleda and facilitates their removal from the body through vamana.

      Also Triphala decoction enema in a dose of 600-900 ml is given in the evening before the Vaman.

      At the day of the procedure, patient should come to the clinic by drinking 1 glass of  milk at home & may bring mixture of curd, jaggery,1 Banana  and salt.This will help to make Kapha docha Uttkat . Vamana should not be given on empty stomach.

      Then the medicines are given, those induce vomiting ( Vamak drug ). Generally madanphal pippali (seed of madanphala) is used for induction of vamana along with vamanopaga dravyas

 The quantity of drug depends on the strength of the patient.
      After administration of vamak drug (Madan phala (Randia dumetorum)-3 Gm + Yashtimadhu (Glycyrrhiza glabra)-2 Gm +Vacha (Achorus calamos)-1 Gm + Honey- 20 ml + Rock salt- 2 to 3 gm  )
      Then the patient should be given External massage ,specially to chest, back, neck  etc and swedana( Nadi Sweda)
      Then first body hair will stand & forehead sweating will appear ,which indicates ,vitiated doshas are starting separation from body tissues .Then there will heaviness in stomach & abdominal discomfort ,which indicates ,Doshas have reached into stomach from peripheral tissues .Lastly there will be too much Nausea ,indicating ,Vitiated Doshas ( Kapha) are ready to be expelled out
      Then immediately start giving decoction of Liquorice ( Jashthamadha) ,2 to 3 glasses at a time .It is administered as much amount the patient can take .Totally for Nice Vaman to take place ,4 to 5 liters is reqiured
      After that, vomiting starts spontaneously.
      Vomitus is collected and measured. Liquids are given repeatedly to the patient up to the complete removal of kapha dosha from the body.

      4 Types of Examinations are done –i) Antiki ( Pitta should be eliminated last ) ii) Vegiki ( usually 6 to 7 big bouts of Vaman are adequate ) iii) Maniki ( Measurment examination is not so important )  iv) Laingiki ( Observe symptoms of Samyak vaman)
      After the complete removal of kapha dosha, there is removal of small amount of pitta dosha from the body.

      At this last point of completion ,patient has bitter taste with some times little burning , in the throat &  mouth. Vomitus is yellow colored. This stage suggests that the process of vamana is successfully completed. After that the liquids should not be given to the patient. During the process of vamana, pulse & blood pressure of the patient should be monitored now & then, to avoid complications.

      Always remember that this procedure cannot be conducted without co-operation of patient so his or her physical and mental power should be in a good status at the every moment of vaman.
