Basti Therapy in Clinical Practice

1) Cervical Spondylosis ---  Tikta Ksheer Basti + Shali Shastik Pinda Swedan 10 days. It relieves pain, spasm .It gives relief for  sensory  & motor symptoms like Tingling Numbness, pain during the movement of neck

2) Backache ---- Sahacharadi Oil Matrabasti 60 ml + Patra pottali Swedan relieves the pain ,stiffness & spasm

3) Ulcerative colitis , IBS, Bacillary dysentery ---  Mocharas Piccha basti – 25 Gm Mocharas+ 100 ml cow milk+ 100 ml water boil till evaporation of  total Water. Many times this is given by Enema pot & drip method. Internally Bilva Avaleha is useful

4) Dry Cough (Allergic cough),Asthma ---  Sesame oil Matra Basti – 60 ml 7 Days give symptomatic relief.

5) In weight gaining- Balaguduchyadi Oil 50 ml Matrabasti 15 days. + Full body Gentle massage with Kshir Bala oil + Internally ShatavariKalpa or Chyavanprash

6) Oligospermia --- Kavach Beej Churna + Shatavari 10 Gm each in 200 ml milk plus 200 ml water and by boiling till water get evaporated+ Phala Ghrutum basti for 15 days improves the sperm count.

7) Chronic Constipation ---  Matra Basti with Chincha Lavan oil is useful & used in a dose of  60 ml for 7days.+ advice about adequate drinking of water + regular walling exercise + Yogic exercises to improve tone of abdominal muscles like Naukasan + Enough roughage in diet

Essentials of Pre treatment ( Purva Karma ) & Complications ( Vyapad)

      Before the Basti following things are essential,

      External Oleation i.e. massage (Abhyanga)- At least on stomach & back.On abdomen it should be more circular & soft .Make sure that there is no serious pathology inside the abdomen loke ulcer,perforation,strangulation,Intusception

      Fomentation- Tube fomentation or fomentation with napkin.
      Anuvasana basti (if patient has excess dryness, then give  2-3 Anuvasana basti)

      Light meal before Anuvasan and empty stomach before Niruha.

      Excretion of the stool and passing of urine.

Symptoms of Proper Effects of Basti

      Timely removal of mala (waste products), Pitta, Kapha and Vata respectively.
      Lightness in the body
      Improvement in digestive power
      Decline in diseased condition

Symptoms suggesting less action of Basti

      Incomplete removal of vitiated doshas and basti dravyas
      Pain
      Heaviness in the body
      Breathlessness, nausea
      Treatment should be done for expulsion of basti dravyas

Complications from incorrect Basti

      There are very few complications in most cases for either Niruha or Anuvasanabasti.
      The most common problems that can happen are -
      Irritation or burning at the anus
      Abdominal pain
      General feeling of uneasiness due to basti in the colon
       General feeling of uneasiness due to psychological reasons
       Headache
      Constipation or diarrhoea
      In most cases these symptoms are rectified

Symptoms suggesting strong action of Basti

      Weakness, body ache, drowsiness, fainting etc. proper treatment should be done to relieve these symptoms. Deepan and Pachana chikitsa is to be used.

After treatment

      Take rest
      Bath with lukewarm water
      Have light diet. Sansarjanakrama is not necessary.
      Use warm water to drink.

Don’t

      Sit for long time
      Vehicle journey
      Sleeping during day time
      Coitus
      Suppression of natural urges
      Having cold water for drinking or even bath ( which can provoke vata)

Basti Types according to the number of Basties implemented

      Karma basti
      Kalbasti
      Yogbasti

In  usual Basti karma, both Niruh and Anuvasana are not implanted serially.

 But then depending upon the serial arrangement of Anuvasana and Niruha and their number, basti is named as Karma, Kal and Yog Basti. 

In these 30, 16 and 8 number of Basties is given respectively.
Usually alternate Niruha and Anuvasan are given. The cycles are followed.

Course of Basti

1)  Yoga Basti:

      (3 Niruha + 5 Anuvasana = 8 bastis) Control to maintain Vata
      1st Anuvasana / 2nd Niruha / 3rd Anuvasana / 4th Niruha / 5th Anuvasana / 6th Niruha /7th Anuvasana / 8th Anuvasana – First & Last 2 Ansvasan

2)  Kala Basti:

      (6 Niruha + 10 Anuvasana = 16 bastis) Control of seasonal Vata

Economics of Basti

      Rubber catheter cost is about 6-7 Rupees per piece.
      Plastic gloves are available at 80 Rupees per 100 pairs.
      Enema pot is about 150 to 175 Rupees.
      Medicated oil cost varies from 350 to 450 Rupees per liter.800 to 1200 rupees per liter
      Generally Basti costs about 150-200 Rupees per day.

Some Other types of Useful Basti – 2

Vaitaran Basti
Indications:
 In Ama associated conditions to relieve the pain e.g. in Amavata  (RA).It reduces the acute inflammation especially relieves the pain & burning.

Ingredients-

Chincha leaves ( Tamarind) --- 40 Gm,
Jaggery- 20 Gm, Rock Salt- 10 Gm,
Cow Urine 160 ml & Sesame oil- 30 ml

It is used for 1 – 3 weeks.

LekhanBasti:
      Herbs like Triphala, Mustha, Kulatha, Sahachar are used for decocotion along with Cow urine or Gomutrasava. Rock salt is also added in it with honey.
      It is given in 500-900 ml dose alternate day. Triphaldi or Sahacharadi oil 60-100 ml are used for Anuvasan Basti.

      It is used in obesity, in hypercholesterolemia & in  PCOD with obesity

a)  Tiktak sheer Basti: 
Indications:---
      Indicated in osteoporosis, osteopenia, cervical spondylosis, lumber disks problem like compression, prolapsed disks,Pathological fracture, hair graying, hair fall, dental caries, for the height gaining.

      Guduchi, Patol, Vasa, Nimb, Kirattikta Bharad total 50 Gm + 800 ml water, boiled till it remains 100 ml followed by 100 ml milk addition and again boild till water gets evaporated+ PanchaTikta Ghee 10 ml to 40 ml.

b)  Piccha Basti
      Indicated in IBS, Bacillary dysentery, Ulcerative colitis etc.
      10 Gm Mocharas i.e. resins (Dink) from Savari in 1 cup of milk i.e. in 80 ml.  It is given sometimes by drip method
       
Types of basti according to the functions performed by Basti
      Shodhan basti – cleansing action
      Snehan basti – Oleation
      Brunhan basti – Nutritive
      Lekhan basti – Scratching of doshas
      Vajikar basti – Increases sexual ability & reproductive functions.

Types According to the intensity of substances used in the basti
      Tikshna basti – Strong substances are used like Cow urine, kshar, kasis Taila
      Mrudu basti – Mild substances are used like honey, ghee etc.

Some Other types of Useful Basti -- 1

a)  Madhutailik Basti
      Madhutailik basti is a sub-type of Niruh basti in which madhu i.e. honey and taila i.e. oils are used in equal or much more quantity.

b)  Yapanbasti
      It is used for children, women, old age persons, for removal of vitiated doshas   and also for the improvement of complexion and voice.
      This Basti provides longevity of human being. Substances used are honey and oil , approximately 90g each + Erand root decoction 180ml + Shatpushpa Churna 20mg +  Saindhav (Rock salt) 10 Gm +  Madanphal Churna 10 Gm.

c)  Siddha basti
      Increases strength, muscle power, improves complexion.
      Substances used – Yava, Kulattha, Pippali, Yashtimadhu, Saindhav, Honey. Generally each substance is taken in 40 gm dose and water is added 4 times and after boiling  till it remains 1/4th it is given in 250 ml dose.

d)  Rajyapan Basti- Cleansing & tonifying in nature.
Indications:
      Indicated in Urinary tract infections, glomerulo nephritis, avascular degeneration or necrosis  of hip joints, Inguinal & Scrotal pain, Musculoskeletal diseases like muscular dystrophy, neurological cases like Multiple sclerosis, Parkinson's disease etc.
      It is recommended in dry allergic cough or in COPDs like emphysema, bronchiectasis, chronic fever, gouty arthritis, irritable bowel syndrome, Knee joint osteoarthritis etc.
      It is used in Auto immune conditions like Ankylosing spondylosis & RA.

Ingredients-

       Decoction of Ashwagandha, Shatavari, Erandamool, Bala in a dose of 240 ml.
      Honey-20 ml.
      Rock salt- 5 Gm
      Sesame oil- 40 ml
      Ghee- 40 ml.
      Meat soup – 160 ml & Cow Milk – 160 ml.
