Shamana Nasya ( Pacifying ) – Indications – Referance -- Ashtang Hrudaya Sutrasthana 20/4

1.  Indicated in diseases due to the vitiation of Blood and Pitta ( Raktaja and Pittaja) like premature wrinkles on face (Akaala vali)

2.  premature graying of hairs (Palita)

3.  Hair fall ( khalitya)

4.  Psoriasis dandruff of the scalp (Daarunaka)

5.  Redness of the eyes (Raktaraaji)

6.  Blackish discoloration on face  (Vyanga)

7.  Neelika

8.  Bleeding from nose = Epistaxis ( Urdhwagata Raktapitta)

Navana Nasya --- This is Sneha Nasya .Only Sneha or medicated sneha is used .

Vacha tail will be for Shodhan or Virechan Nasya .

Anu Tail will be used for Shaman Nasya .

 Bala Tail will be used for Bruhan Nasya

Which Navan Nasya does Bruhan or Nourishment ,that type is called Nasya ,by Sushrut .Referance Sushrut Chikitsa Sthana 40/18 .Sushrut said Sneha Nasya is given to strengthen the parts like neck,shoulder & chest .

Charak – Types of Nasya ( Referance –Charak Siddhi Sthana  9/90 to 93) --- 1) Navan 2) Avapidak 3) Dhampan 4) Dhuma 5) Pratimarsha

As Dhumapan is done through Nose ,Charak has included Dhuma ,as a Type of Nasya ( Dhuma is  not explained by Vagbhata)

Dhumpan will be discussed in another blog .Here we have some brief about Dhooma Nasya

Dhooma nasya

1.  Medicated fumes are inhaled through the nasal passages and are eliminated through the oral route.

2.  Types-

1)  Prayogika –

 Can be taken daily as a part of Dinacharya                                        

2)  Snehika –

In this method we used Snigdha drugs

3)  Virechanika

Nasya – Classification

According to action ( Sanskrit word is Karmukata)

( Referance of Vagbhata – As Hr Sutra 20/2)

i)            Brinhana – Nourishing – It is also called as Tarpan Nasya.To control Dhatukshyajanya Vata Prakop .Degenerative pathology like senile dementia or Parkinsonism ( Deficiency of Dopamine Neurotransmitter) or Mysthenia Gravis ( Deficiency of Ach Neurotransmitter),Bell’s palsy,Dryness in nose,throat etc, Migraine  in thin female patient .Usually plain or medicated ghee is used

ii)           Shamana – Pacifying Nasya .Used in moderate Dosha Prakop or weak patients ,where Strong shodhan can not applied

iii)         Virechana – or Shodhan or Purification Nasya – It is used when Dosha Prakop is severe & patient is strong & stout .Conditions are treated like heaviness in the head ,Chronic sinusitis,Anosmia ( Loss of smell sensation) ,Thyroid or other tumor like growths

According to Dose

i)            Marsa Nasya :-- Dose is 8 to 32 drops in each nostril .It is used when Dosha prakop is moderate or severe & patient is strong & stout .Ghee or oil is used for marsha nasya treatment

ii)           Prathimarsa Nasya :-- Dose is only 2 drops in each nostril .This is very soft & gentle Nasya .Can be used as a preventive measure or Dosha prakop is mild .This can be used in weak, delicate people, in children, old people, females .It is also called as aajanma marana satmya nasya ( meaning is tolerable from birth to death) .Because there isn’t any fear of side effect as the dose is very low i.e. two drops in each nostril. Thus we can perform it in any time and at any age of person.

According to the substance used for Nasya  

i)            Avapeedaka Nasya –Avapida nasya where the paste of the drug(kalka) is squeezed into the nostril and the juice (extract) of a drug is administered through the nasal passages. This is teekshna Nasya .In this nasya type substance is used other than Sneha dravya.

Chakrapani ,critics on Charak said Avapeedak is of 2 types Shodhan & Stambhana

Types of Avapida nasya: –

a)  Shodhana.-- Eg. Nasya of palandu swarasa

Indicated in  manasa vyadhi like Unmad Hysteria or ACR = Acute Conversion Reaction) , Apasmara ( Epilepsy) , Murchha, krimi ( Maggots) , Ardhavbhedakadi shiroroga ( Migraine) ,coma,poisonong

         b)Stambhan  --  Eg. Nasya of durva swarasa, cow’s milk, sugarcane juice ,concentrated sugar water ,milk

Indicated in  Nasagata Raktapitta ( Epistaxis) ,in weak patients

    

ii)           Pradhaman Nasya – This is also called as Dhmpan nasya .Pradhaman means to blow .This is only of Shodhan type. In this method dry powders (rather than oils) are used to blow into the nose with a tube attached to rubber bulb like that of attached to BP apparatus .Other wise paper roll can be used to blow the powder .

Dose is 3 pinch powder .Pradhamana Nasya is a type of virechanic nasya (cleansing nasya).Traditionally, powders such as vacha, Satala or Shikekai powders are used in this process.

Videha ,an ayurvedic scientist ,explained other time of Pradham .Take very fine powder of medicinal herb  in very thin cloth & just smell intermittently .Vacha powder or Ajwayan or Owa powder can be used in this way in running water from nose ( Cold or Pratisyaya) .This is comparatively safe method than blowing powder in to the nose

iii)         Navan Nasya ---- This is Sneha Nasya .Only Sneha or medicated sneha is used .Vacha tail will be for Shodhan or Virechan Nasya .Anu Tail will be used for Shaman Nasya & Bala Tail will be used for Bruhan Nasya

Introduction of NASYA KARMA

When Uthamanga gets fire –
 You need a Nasal Shower…..

Uttamanga means Parts above clavicle or collar bone.Mainly it refers to ENT ,Ophthamic ,Head .Nasya is done to nourish these organ ,strengthen these organ,prevent diseases of these parts .

If we talk about Panchakarma as a Shodhan ,then Nasya is supposed to do purification or cleansing of these parts .Nose is the Main gate way to enter the Brain region .Note the olfactory pathway from cribriform plate of Ethmoid bone in the upper part of nose .Ayurveda says ,Nasa hi Shirso Dwaram’.So action of Nasya dravya ,medicated oils or Ghees ,which are fatty substances ,will be seen on centers of Sensory & Motor organs ,Intelligence area in frontal lobe ,Visual centers as numbered 17,18,19; Auditory centers as 21,22,41,42; Wernick’s area ,Broca’s area of sppech.This is why Nasya helps in Neurological problems like Hemiplegia, Bell’s Palsy, Parkinsonism, Alzheimer’s disease, senile demensia, Tremors, Low IQ etc.

Apart from medicated oils & Ghee ,sometimes powders, fresh juice, smoke are also used for Nasya

Definition of Nasya ---‘Nasayam praniyamanam aushadham nasyam’( Medication given by nose is called as Nasya)

Nasya is a procedure wherein medicaments are instilled into nasal cavity to expel doshas pertaining to Uttamanga

The nasya dravya (medicine) acts by reaching at  'Shringataka marma‘ -a main vital point situated on the surface of the brain corresponding to the nerve centres, which consisting of nerve cells and fibres responsible for the function of speech - Broca's Centre,as wel as the center of vision, hearing, taste and smell

Purvakarma(Pre-purification Measures) --- There are mainly 2 ways to apply Nasya .One is after other Panchakarma procedures like Vaman,Virechan,Basti & other way is applying Nasya as alone Therapy .If it is followed by other panchakarma ,there must be rest period of one day

In any ways following points are same

Patient must have his bowels opened i.e there should be Anuloman gati of Apan Vayu. This will facilitate the action of Nasya on Pran & Udan vayu

Patient is asked to empty the bladder ,before starting procedure

If patient is hungry ,he make take non irritating light food

Brushing the teeth ,mouth wash ,gurgling, will be better to be done

   Prior to nasya karma, snehana and swedana should be done to the patients Face, Forehead, Head, Ears and Neck. Apply suitable oil like Sesame oil or Kshirbala oil to these parts like facial massage for 10 minutes.

Fomentation can be done with the vapours ,coming from Napkin dipped in boiled water or through Nadi sweda or hot water bag .

This prior snehana (oleation) and swedana (sudation or sweating) help to loosen the adhesive doshas, thereby facilitating the subsequent elimination.

Pradhan karma (Main Procedure of Nasyakarma)

After the Purvakarma the patient is asked to lie down in a bed with his hand and legs kept straight.

His head is maintained at a lower position by keeping the pillow below the neck, Shoulder. (head low position)

Another option is, ask patient to sit in a special chair like Dental chair .Then neck can be rested on cushion, to keep face in lower level with nose up

This position will facilitate the direct passage of the drug in to the brain

Warm the medicines like Anu tail or Pnchendriya vardhan tail by placing it in hot water bath.

Then therapist or vaidya will stand on the backside of the head of patient & will ask the patient to close the eyes gently (To avoid oil part getting touch to conjunctiva ,which can cause irritation & pain in the eye) .Then by closing left nostril by thumb ,4 to 8 drops are put in right nostril with dropper .Same procedure is adopted by closing right nostril for putting the drops in left nostril .

After putting the drops ,if you are doing Shodhan Nasya & Patient is stout & sturdy ,then patient is asked to inhale the medicines strongly & forcefully .Then cough ,mucus,lot of secretions come out .Patient is asked to spit them out in a bowl ,which he is provided .This is usually followed method of Kerala

If Nasya is Nutritive time & in a very low dose is given as Pratimarsha ,then patient must inhale very gently & softly ,after putting the drops .No mucus or Kapha or secretions  are expected to spit out .Little irritation of throat can occur ,because some part of medicinal oil may escape in throat .Patient is asked to do warm water gurgling by which irritation of throat goes off

Pradhaman Nasya ---This  is Sanya Prabodhan Nasya .Senses get stimulated & are awakened .But this treatment should be given by Expert vaidyas only & should also perform in strong & stout patient .If these due precautions are not taken ,there may be complications like Epistaxis,Giddiness .Calamus ( Vacha) or Black papper powders can be used

Vapour Nasya – Medicated vapors are created by putting leaves of Eranda or Nirdundi or wet ginger in boiling water .Then by putting Towel on head & face, patient has to inhale these fumes

Paschat karma (Post management) ---

Mild fomentation ,is once again given on forehead, face, throat & neck .Patient’s hands & palms are rubbed .This stimulates the action of Nasya medicines .Patient can wash the face by luke warm water & then gurgle with warm water .After purification Nasya ,always instill some drops of Cow’s Ghee

Patient should be given following advice –

No  shower or bath or swimming till next 12 hours

No exposure to very hot sun ,No wind, No rain ,No AC

No cold drinks or No Ice creams

No too much talk or Physical activities

No day time sleep & not keeping awake till late at night

Action of drugs used in Nasyakarma

1.  By general blood circulation increases, after absorption through mucous membrane.

2.  Direct pooling into venous sinuses of brain via, inferior ophthalmic veins.

3.  Absorption directly into the cerebrospinal fluid.

Benefits of Nasya

Nasya can prevent diseases of Ear, Nose, Throat, head, eye etc if Nasya is done regularly

Nasya can be a part of Management of various diseases like Hair fall, Baldness, early graying of hair

As nasya can purify & nourish all parts of brain, it is useful in headache, Migraine, Hemiplegia , Facial Paralysis, Aphasia

Post Raktamokshan Advice –

Patient should avoid ----  Pitta Vitiating food & Life style .No excess Pungent,Sour & salty food like Vada Pav with green chilly,eating pickles regularly ,Non veg food,Spicy Kolhapuri & Punjabi dishes,Chilly or Tomato sauces,Chatni of groundnut or green chilly ,Curd ,Excess south Indian dishes like Dosa,Uttapa ,Idli .Hot potency food & beverages like Tea,Coffee,Alcohol .Antagonistic food like mixing of food with acid & alkali like milk shake ,Fruit salad,mixing banana & milk

Patient should avoid --- Excess exposure to too cold or too hot,exposure to hot sun,anger,jelousy,Day time sleep ,Sex indulgence

Benefits of Raktamokshan --- Releasing small amount of blood remove congestion due to Ama ( Endogenous or Exogenous Toxin) in body channels & stimulate body’s own mechanism of detoxification

Experienced cases:

1.  A 42 yr lady was operated for right leg debridement. After that she has leg swelling below knee, hardness below knee, non healing ulcer at ankle joint, no sensation at foot and finger and difficulty in walking due to increased wt of that leg. She was suffering from these complaints  from six month.                               In the first visit we apply two leeches. After bloodletting swelling reduced, skin color change towards red, heaviness reduced. After 1 wk we apply 4 leeches. At this time after bloodletting swelling as well as heaviness reduced and she got sensation over the foot. She had undergone with dhara sweda of panchavalkala decoction, leech settings and some internal medicines. Within two months she got 70% result.

2.   44 yr lady of herpes zoster is treated with siragata raktamokshana ( Venesection) in the first visit. About 40 ml blood was removed. Instantly she got the relief from burning sensation. After that leach application was done three times. Two leaches every time .simultaneously internal medicines were continued. She cured within 2 wks.

3    40 yr lady was suffering from bleeding piles from 4 to 5 yrs. She came with bleeding piles, swelling and intolerable pain at the anus. Two leaches were applied at inflamed pile. At the end of bloodletting she got the 50% relief from swelling and pain. We repeat the leach therapy every 5 day twice and some internal medicines were given.From last 4 months she had no complains
